Provides community-based housing for women and children who have experienced long-term or chronic homelessness and have been diagnosed as having a physical or developmental disability, a severe mental illness, substance use disorder or HIV/AIDS.

Must have/meet: * Individuals experiencing homelessness * Mental disorder diagnosis or * Substance abuse problem or * HIV/AIDS diagnosis * Able to live independently * Able to manage medical needs * Provide a negative urine screen within 30 days * Willing to work toward independent living * Cannot be a registered sex offender * Cannot have history of violent behavior
